The satrical disclaimer for South Park appears at the start of most episodes just before the title sequence.
The disclaimer is an in-jokes to the program. It references to facts such as the show using real life events and people as well as fictional. The line stating the show should not be viewed by anyone does apply to some countries who have banned the series altogether.
The format of the disclaimer has been tweaked slightly over the past few years. It was designed in a different manner for the first two seasons along with half of the third. The disclaimer was later redesigned into a smarter format that is used today.
An episode that did not feature the disclaimer was "Pip". The movie, "South Park: Bigger, Longer and Uncut" didn't use it either.
